Jukebox Health Acquires Braided Health to Expand AI-Driven Integrated Care Platform for High-Needs Duals

Augments Jukebox Health’s home safety and independence ecosystem to help Managed Care Organizations modernize legacy care management systems and lower total cost of care for high-need, dual-eligible populations.

NEW YORK, April 1, 2026 /PRNewswire/ — Jukebox Health, a leader in advancing safety and independence at home, today announced the acquisition of Braided Health, a care management technology company serving high-need, dual-eligible Medicare-Medicaid populations.

With this acquisition, Jukebox expands its platform to support health plans in managing total cost of care for complex populations by connecting care management with in-home clinical insight and intervention.

Medicaid covers nearly 90 million Americans, with dual-eligible members driving a disproportionate share of spending. Managed care organizations are under increasing pressure to identify risk earlier and better manage costs for their most complex members. That responsibility sits with frontline care managers, who rely on legacy systems not built for the challenges of dual-eligible populations.

Braided Health addresses this gap with its AI-powered “sidecar,” which works alongside existing platforms to surface actionable insights without requiring plans to replace their current software. Jukebox acts on this intelligence by deploying its interdisciplinary network of clinicians to the home to validate needs, supported by a service network capable of immediate action.

What sets Jukebox apart is its ability to operate directly in the home, where most of these cost drivers originate. By deploying clinicians into members’ living rooms, Jukebox captures findings that traditional programs miss and has built a proprietary dataset linking functional health and environmental conditions to financial outcomes. Jukebox translates these signals into targeted interventions, such as enabling independent bathing to reduce ongoing personal care needs or deploying telehealth and remote patient monitoring to better manage higher-risk patients. This approach has demonstrated a strong, repeatable ROI for plan partners by mitigating risks that drive an estimated $35B in annual avoidable personal care utilization, hospitalizations, and institutional placements.

Braided Health’s CareAssistâ„¢ platform extends Jukebox’s model into the clinical workflow. This technology works alongside existing systems to streamline caseload triage, alert staff to emerging risks, minimize documentation burden, and surface data from the home and third-party sources. Together, these capabilities give care managers the insights, tools, and structure to focus on high-impact member engagement.

“For plans managing complex dual-eligible and HCBS populations, the challenge isn’t identifying risk – it’s acting on it in a meaningful way,” said Romanos Fessas, CEO and Co-Founder of Jukebox Health. “This acquisition allows us to connect what’s happening at home with the broader care management process, so interventions can happen earlier and with greater impact.”

The acquisition also brings two seasoned industry leaders to Jukebox Health. Jeff Grahling, former CEO of Braided Health, brings deep health plan experience across Centene, Cigna, and Wellpoint. Emily Olson, RN, adds clinical leadership across behavioral health and care team operations.

“Braided was built to empower care managers with more personalized insights to guide higher value decision-making,” said Grahling. “Together with Jukebox, we’re positioned to build something much more powerful: linking health plan workflows with in-home clinical data to drive meaningful outcomes.”
