MCLEAN, Va., Oct. 1, 2025 /PRNewswire/ — Somatus, a leader in specialist-led primary care management for patients with, or at risk of, kidney or heart disease, today announced the results of a new study demonstrating that its care model delivers a 13% relative reduction in mortality for patients with kidney disease. The finding, generated from a rigorous, propensity-matched study of more than 165,000 patients, was statistically significant (p<0.001) even after controlling for numerous clinical acuity factors associated with mortality. This result complements Somatus’ track record of achieving in its owned and operated outpatient dialysis centers a 20% improvement in mortality relative to the national rate.
“Somatus’ patient-centered clinical model is purpose-built to ensure patients receive the most impactful interventions needed to meaningfully improve outcomes,” said Dr. Ikenna Okezie, Chief Executive Officer and Somatus Co-Founder. “We founded Somatus on the principle that it is possible to do well by doing good. From Somatus’ inception, our goal has always been to extend lives and improve quality of life for patients. I am delighted to see our focus on technology-powered multidisciplinary care and provider partnerships is delivering meaningful and scaled results for patients, providers and payors.”
Somatus continues to expand its comprehensive model of care, which integrates:
- Value-based care and provider partnerships that align incentives and improve outcomes across health systems and physician groups – currently contracting with 5,500+ specialty and PCP providers, including 1,500+ nephrologists (about 15% of all nephrologists in the U.S.).
- Somatus Medical Group (SMG), a safety-net telehealth solution that connects patients who lack access to care with nephrologists and internists for episodic care and transitions of care support.
- Somatus Transition Concierge, where Somatus’ nurses and patient health advocates partner with hospitals to ensure timely patient transitions of care, reducing avoidable hospitalizations and supporting continuity of care.
- In-home multidisciplinary care teams that deliver personalized, whole-person care tailored to the unique needs of patients who need it most.
Somatus’ rapidly growing footprint is backed by measurable impact, including a statistically significant 17% reduction in the medical cost of care and a 31% improvement in hospitalization rates for assessed patients compared to unassessed patients in a national payor designed propensity-matched, difference-in-difference study.
By delivering this model nationwide, Somatus is working collaboratively with payors, providers and patients to transform chronic condition care and build a more sustainable healthcare system.
