MyDirectives and KanTime Announce Strategic Partnership to Bring Digital Advance Care Planning Directly into KanTime’s EHR

Integration of MyDirectives for Clinicians™ into KanTime’s EHR empowers post-acute providers to securely access and share advance care plans nationwide

DALLAS–(BUSINESS WIRE)–#acpMyDirectives, the leading provider of digital advance care planning (ACP) solutions, and KanTime Healthcare Software, the industry leader in enterprise software for post-acute care, today announced a strategic partnership to embed MyDirectives for Clinicians™ directly into the KanTime EHR. This integration will empower KanTime’s home health, hospice, palliative, and home care providers to seamlessly access and share patients’ advance care plans, delivering goal-concordant care, improving patient and family satisfaction, and avoiding unwanted treatments or interventions.

With the integration underway and full availability expected in the coming weeks, KanTime clients can gain instant access to critical ACP documents, including advance directives, POLST forms, Five Wishes®, and mental health advance directives (MHADs), all without leaving the KanTime workflow. By ensuring these documents are available across care settings, clinicians can honor patient preferences during transitions of care—especially critical in emergency department and EMS scenarios.

Activating the integration is simple. The heavy lifting has already been done behind the scenes, and KanTime clients can enable MyDC with minimal effort by simply contacting KanTime to activate their organization’s MyDC license.

Through this partnership, KanTime users will also benefit from anytime, anywhere access to ACP documents via the A|D Vault Exchange™, MyDirectives’ propriety nationwide registry. Built on a HITRUST-certified and SOC 2 Type II–audited platform, the A|D Vault Exchange ensures that critical documents can be securely accessed and shared across healthcare organizations, HIEs, and emergency settings, no matter where.

“Our mission at MyDirectives is to provide the ability for every person’s voice to be heard in their care, no matter where they are,” said Scott Brown, president and CEO of MyDirectives. “Embedding MyDirectives for Clinicians directly into KanTime’s platform exemplifies innovation and interoperability at work—eliminating silos, improving clinician workflows, and ensuring that critical advance care information is available the moment it matters most.”

“KanTime has always been committed to helping our providers deliver exceptional patient-centered care,” said Sundar Kannan, CEO of KanTime Healthcare Software. “This partnership with MyDirectives ensures that our clients are able to honor their patients’ care and treatment preferences, improve safety, and enhance satisfaction, all while maintaining seamless workflows. It’s an innovation that benefits patients, families, and providers alike.”

Attendees of the Annual KanTime Experience will have an opportunity to learn more about the integration and see MyDirectives in action. MyDirectives is proud to sponsor, exhibit, and present at the event, including the session, “From Paper Trails to Patient Goals: Digitizing Advance Care Planning for Real Impact,” led by MyDirectives’ Jessica Zan, RN BSN MAMFC, VP of Clinical Implementations & Success, and Kathleen Taylor, Director of Client Engagement & Education.
